What Ontario's FHO Model Means For You
With the Ontario Physician Services Agreement (PSA) having expired in 2024, the Ontario Medical Association (OMA) and the Ministry of Health (MOH) are negotiating a new agreement that could reshape how family doctors are compensated.
The proposed FHO+ model, currently under arbitration, represents a major step toward recognizing the full scope of work physicians perform — from patient visits to charting and chronic care management. A ruling is expected this fall.
Why FHO Matters
The FHO+ model is an evolution of Ontario’s Family Health Organization (FHO) structure — not a replacement. It’s designed to:
- Reduce administrative burden
- Reward physicians for indirect patient care
- Stabilize clinic operations and improve income predictability
- Support recruitment and retention across Ontario
Key Highlights of the Proposed Model
Time-Based Payments — Physicians will be eligible for hourly compensation (up to $80/hour, billed in 15-minute increments) for:
- Direct patient visit
- Telephone consultations
- Charting, billing, and lab reviews
- Roster management and care coordination
Increased Shadow Billing & In-Basket Fees — In-basket payments will rise to 30–50% of full FFS rates, creating higher revenue for services already being provided within a FHO.
Improved After-Hours & Leadership Compensation — After-hours premiums will increase from 30% to 50%, and leadership stipends (GMLP) may grow up to four times current levels.
Enhanced Patient Rostering Incentives — Bonuses for patient attachment will rise by $100–$270 per patient, with additional incentives for new graduates and rural physicians.
Simplified FHO Formation — Minimum group size reduced from three to two physicians, and 240 new FHO spots will be available annually — creating more opportunities to join or form your own FHO.
